Horizon OT encourages feedback from all people we work with. This includes, but is not limited to, people with disabilities, families, carers and service providers.

If you are submitting a complaint, our office will reach out to you as soon as possible. Horizon OT takes all feedback and complaints very seriously, as it is invaluable information that helps us grow as a service provider.
