Donna Mills started her private practice in 1999 in Caloundra, on the Sunshine Coast and has expanded to employ a team of experienced and caring Occupational Therapists. Horizon Occupational Therapy Services provide assessment and management of adults with difficulties participating in their everyday activities. We provide services in your own home, from Bribie Island to Gympie.
Our Services
NDIS
The National Disability Insurance Scheme (NDIS) is a program that provides people who have a permanent and significant disability with funding for supports and services. We can assist you to pursue your goals and increase your independence and community participation.
Support at home program
The Support at Home program was introduced 1 November 2025, replacing the Home Care Packages (HCP). Horizon OT broker to several Support at Home providers to provide Occupational Therapy intervention for their participants. Discuss with your provider for a referral to our services.
Equipment Prescription
In assisting people to improve their function, we as Occupational Therapists are experts at prescribing specialised assistive equipment to assist you in performing your everyday activities. We can assist you with equipment from wheelchairs to kitchen equipment to help you be as independent as you can.
Department of Veterans Affairs
The Department of Veterans' Affairs (DVA) in Australia offers occupational therapy services under the RAP (Rehabilitation Appliance Scheme) to support veterans in maintaining independence and safety in their daily activities. Discuss with your GP for a referral to our services.
hOME SAFETY AND FALLS ASSESSMENT
Falls can have a big impact on your health and well-being. We will work with you in your home to identify your risk and put into place a strategy to improve your health.
Driving Assessment
If an OT driving assessment is appropriate for you, we will screen you for physical, cognitive and experiential factors that might influence your safe driving ability. Then we complete an on-road assessment.
What is Occupational Therapy?
Occupational therapy is a client-centred health profession concerned with promoting health and well being through occupation. The primary goal of occupational therapy is to enable people to participate in the activities of everyday life. Occupational therapists achieve this outcome by working with people and communities to enhance their ability to engage in the occupations they want to, need to, or are expected to do, or by modifying the occupation or the environment to better support their occupational engagement.
